Fee Calculation

13% HST Included in Price

 1 Dancer - $1380 per year ($115 per month) 2 Dancers - $2070 per year ($172.50 per month) 3 Dancers - $2415 per year ($201.25 per month)

*Price includes 6-8 hours of dance per week, 2 competitive costumes (1 jazz, 1 tap), 2-3 dance competition entry fees, 1 tracksuit, free classes outside of competition classes (costumes extra for extra classes)
(price does not include photos or recital tickets)
*No discounts are applicable to special competitive rates

Payment Plan 1 - Full year
 Cheque or Cash Payment for entire year (Dated October 1, 2011)

Payment Plan 2 - 2 Installments
 2 Cheques or 2 Cash Payments for entire year (Dated October 1, 2011 and February 1, 2012)

Payment Plan 3 - Monthly
 9 Cheques or 9 Cash Payments (1st of each month for 9 months)

Waiver

This agreement affects your legal rights. Read it carefully.

I understand that participating in a dance or exercise class is a potentially hazardous activity. I should not participate in such activity unless I am medically able.
I agree to abide by any decision of the instructor relative to my ability to safely participate in such activity through Hype Danz. I assume all risks associated with
participating in such activity including but not limited to falls, contact with other participants, slipping, etc. all such risks being known and appreciated by me.
Having read this waiver and knowing these facts and in consideration of Hype Danz accepting me into such class or activity, I , for myself and anyone entitled to act
on my behalf, do waive and release True Warrior Martial Arts, Hype Danz, its employees, contractors and owners, including Alison and Darrell LaFrance, and
Margarita Camera from all claims or liabilities of any kind arising out of participation in this activity.

You the member, are aware that there are risks associated with participating in Fitness activities and exercise. Your participation is completely voluntary, and you freely
accept and fully assume all responsibility for all risks, and all possibilities of personal injury, death, property damage or loss to yourself or any other person as a result
of your participation in fitness activities. You and your heirs, next of kin, executors, administrators and assigns agree:
a. To waive all claims, known or unknown, that you have or may have in the future against Hype Danz and Margarita Camera including their owners, officers, directors, agents,
employees, volunteers, business operators, independent contractors and site property owners or lessees.
b. That Hype Danz and Margarita Camera is not liable or reponsible for any damage to, loss or theft of your property.
c. To release and forever discharge Hype Danz and Margarita Camera from all liability for any personal injury, death, property damage or loss resulting from your participation in
fitness activities due to any cause, including but not limited to negligence (failure to use such care as a reasonably prudent and careful person would use under similar circumstances),
breach of any duty imposed by law, breach of contract or mistake in error of judgment of Hype Danz and Margarita Camera.
d. To be liable for and to hold harmless and indemnify Hype Danz and Margarita Camera from all actions, proceedings, claims, damages, costs, demands, including court costs on a
solicitor and own client basis, and liabilities of whatsoever nature or kind arising out of or in any way connected with your participation in fitness activities.

As a student of Hype Danz, I understand that photographs and/or videos of myself and/or my likeness may be used for promotional purposes and for safety purposes.
My signature hereunder (or that of a parent or guardian for dancers under 18), gives permission to Hype Danz and Katherine Aczel Photography to so at their sole discretion.
